Petition the Saint Paul York Rite
I being a Mason of good standing in the below mentioned Lodge and under the jurisdiction of the Grand Lodge of Minnesota, certify that I reside within this jurisdiction and do respectfully petition for the degrees and Orders of the York Rite of Freemasonry, as indicated below, promising to conform cheerfully and strictly to all bylaws, rules and regulations of the national, state, and local bodies of the York Rite of Freemasonry. I hereby affix my signature or representation thereof to this petition and state that all of the information is true and wish to petition the named bodies of the York Rite.
